To keep your wait as brief as possible we ask that you arrive 15 minutes prior to your scheduled appointment time in order to fill out the necessary forms we require for your visit.

If you prefer, using the access code you were given when you scheduled your appointment, you may log into our Patient Portal and down load copies of the medical history and physical form and the current medication list forms to complete prior to your visit.

Below you will find selected procedure consent forms which you may also complete and bring with you. If you do not see a consent form for the treatment you are having we will provide you with that document when you arrive.

As a courtesy to other patients, should you need to reschedule your appointment please contact us 24 hours in advance of your scheduled appointment time.
